What is SULT1A1?
SULT1A1 (sulfotransferase 1A1) is an enzyme found in hair follicle cells and skin tissue. Its job in the context of minoxidil is simple but essential: it converts minoxidil (the inactive prodrug) into minoxidil sulfate, which is the pharmacologically active form that actually stimulates hair growth. Without adequate SULT1A1 activity, topical minoxidil largely passes through the scalp without being activated — and nothing happens.
This conversion step is the rate-limiting factor in topical minoxidil's efficacy, and SULT1A1 activity varies dramatically between individuals based on genetic polymorphisms. Research estimates that 30–50% of the population has low enough SULT1A1 activity to significantly impair topical minoxidil response.
Why this matters so much
Standard of care for hair loss has historically been: try minoxidil, wait a year, assess results. If it didn't work, move on. The SULT1A1 test changes that paradigm. Instead of spending 12 months on a treatment that biology may have already determined won't work for you, a simple patch test can tell you in advance whether topical minoxidil is the right route — or whether oral minoxidil (which bypasses the SULT1A1 conversion step entirely, since it's already absorbed systemically) is a better starting point.
How the SULT1A1 test works
The test used by Hair Scripts — the Hair Scripts Response Test — is a topical patch test performed at home. A small amount of minoxidil solution is applied to the forearm and left for 30 minutes. The local skin reaction (a visible color change caused by vasodilation in proportion to SULT1A1-driven minoxidil sulfate production) is then photographed and submitted through the patient portal. A licensed clinician grades the result on a 1–4 scale.
This approach measures functional SULT1A1 activity in your actual skin tissue — not just a genetic prediction. Genetic testing can identify SULT1A1 polymorphisms, but functional enzyme activity is what determines real-world minoxidil conversion, and functional tests are considered more clinically meaningful.
What the grades mean
Grade 4 (Strong responder): High SULT1A1 activity. Topical minoxidil will likely convert efficiently. Higher-concentration compounded topicals (8–10%) are appropriate.
Grade 3 (Moderate responder): Adequate enzyme activity. Topical minoxidil at 5% or compounded concentrations is expected to be effective, though results may take longer than in Grade 4 patients.
Grade 2 (Borderline): Marginal activity. Topical minoxidil may produce limited results. Oral minoxidil, which bypasses the conversion requirement, is often recommended alongside or in place of topical.
Grade 1 (Likely non-responder): Low SULT1A1 activity. Topical minoxidil is unlikely to produce meaningful results. Oral minoxidil becomes the primary recommendation. Other agents (finasteride, dutasteride, spironolactone, platelet-rich plasma) may be added depending on the patient's profile.

A Grade 1 or 2 patient who was hoping to avoid oral medication now has a clear rationale for the conversation. A Grade 4 patient knows a high-potency compounded topical is worth investing in. And both patients avoid months of trial and error.
