1. Who These Terms Apply To

These Terms of Service (“Terms”) govern your use of the Hair Scripts website and telehealth platform (“Service”), operated in Arizona. By using the Service, you agree to these Terms. If you do not agree, do not use the Service.

You must be at least 18 years old and a current resident of Arizona to receive clinical services through Hair Scripts.

2. What Hair Scripts Is (and Is Not)

Hair Scripts is a telehealth platform that connects patients with independent, Arizona-licensed clinicians for asynchronous evaluation and treatment of hair loss conditions.

Hair Scripts is not: an emergency service, a replacement for your primary care provider, or a substitute for in-person evaluation when in-person care is clinically required.

If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.

3. Telehealth Consent

By submitting your intake, you consent to receiving telehealth services asynchronously — meaning your clinician reviews your submitted information (questionnaire responses, photos, and test results) without a live video or phone appointment.

You understand that:

• —Telehealth has limitations compared to in-person care. Your clinician may not be able to diagnose all conditions remotely.
• —Technical failures (internet outage, file upload errors) may delay your consultation.
• —You have the right to withdraw from telehealth care at any time without affecting your right to future care.
• —Your clinician may determine that in-person evaluation is necessary and decline to prescribe.

4. Prescriptions and Medications

Prescription decisions are made solely by the licensed clinician assigned to your case. Hair Scripts does not guarantee that you will receive a prescription.

You understand that:

• —Medications prescribed through Hair Scripts are compounded — they are custom-formulated using FDA-approved active ingredients, but the final compounded product is not FDA-approved.
• —Some uses of these medications may be off-label. Your clinician will explain this as part of your care.
• —Finasteride and dutasteride can cause birth defects. Women who are pregnant, trying to conceive, or breastfeeding must not use these medications. Do not handle crushed or broken tablets if you are pregnant.
• —Oral minoxidil can affect blood pressure. Your clinician will screen for contraindications before prescribing.
• —Results vary. Hair restoration is not guaranteed. Typical timelines for visible results are 3–6 months.

5. The Hair Scripts Response Test

The Hair Scripts Response Test is a patch test kit used to assess SULT1A1 enzyme activity. It is not a diagnostic medical device cleared by the FDA for this purpose. The test result is one data point your clinician uses to inform treatment decisions — it does not constitute a definitive diagnosis.

A small number of people may experience minor skin irritation from the patch. If you experience significant irritation, redness, swelling, or difficulty breathing, remove the patch immediately and seek medical care.

6. Accurate Information

You agree to provide accurate, complete, and truthful information in your intake form, including your medical history, current medications, and health conditions. Providing false information may result in clinically inappropriate treatment and is a violation of these Terms.

7. Payment and Refunds

Consultation fees, if any, are charged at the time of submission. Hair Scripts does not bill insurance. Compounded medication pricing is provided at the time of prescription.

Refunds are not available for completed clinical consultations. If your prescription cannot be filled due to a pharmacy issue, Hair Scripts will work to resolve the situation or issue a credit.

8. Intellectual Property

All content on this site — including the Hair Scripts brand, Hair Scripts Response Test name, formulation concepts, quiz structure, and copy — is the property of Hair Scripts and may not be reproduced without written permission.

9. Limitation of Liability

Hair Scripts is not liable for adverse reactions to prescribed medications when those medications were prescribed appropriately based on the information you provided. To the maximum extent permitted by Arizona law, Hair Scripts' liability is limited to the amount you paid for the consultation.

10. Governing Law

These Terms are governed by the laws of the State of Arizona. Any disputes shall be resolved in Arizona courts.

11. Changes

We may update these Terms at any time. Continued use of the Service after changes constitutes acceptance of the new Terms.

12. SMS and Text Message Policy

Consent to Receive SMS. By providing your mobile phone number, you consent to receive transactional text messages from Hair Scripts related to your healthcare. Message types include:

• —Appointment and consultation status updates
• —Prescription status notifications
• —Test kit shipping and result reminders
• —Account security verification codes

Message Frequency. Message frequency varies. Typically 2-5 messages per consultation cycle.

No Marketing SMS. Hair Scripts does not send marketing or promotional text messages. All SMS is transactional and related to your healthcare.

Opt-Out. Reply STOP to any message to opt out of SMS communications. Reply HELP for assistance. Opt-out does not affect your right to healthcare services — we will communicate via email or patient portal instead.

Carrier Fees. Standard message and data rates from your mobile carrier may apply.

Privacy. SMS messages will never contain protected health information (PHI). Messages will be limited to notifications such as "Log in to view your results" or "Your test kit has shipped."
